I found out some interesting information!  "Brahms Paraphrase" as it is titled, was originally arranged by Ivan Ditmars for the Star Trek Season 3, Episode 19: 'Requiem for Methuselah'.

In the episode, Spock states that it is recognizable as Brahms original work, but that it was never known to man (ie: unreleased) ..

Looks like Ivan used bits and pieces from a variety of Brahms arrangements and came up with what you hear Spock play!
